Traceable adjustments, not silent balance edits

Refunds reverse unused PAYG credits proportionally to the refunded amount. When a refunded or disputed credit has already been consumed, the account is placed into support review rather than rewriting history.

All credit changes use immutable ledger entries. Corrections use compensating entries and administrator adjustments require a reason.
